Appointments need to be requested. There will be a form that parents needs to complete to see if the child or teenager qualifies for this services. Parents will be contacted to confirm appointment
Insurance and Billing
The ADHD clinic for super minds accepts patients from certain insurances and cash payments for services. The insurances accepted are Aetna, assured benefit, blue cross/ blue shield, Cigna, GPA, Humana, and UMR. Payment is expected in full by the time of service. For patients without insurance, or who decide not to use their insurances, the fees are: New Patients initial ADHD evaluation $300, and for follow ups $175. For patients using insurance, there will be a one time additional charge of $100 for all first time consultations. This fee includes the creation and review of patient medical history, and also the immediate coordination of care selecting medications, letters for school, and a follow up call on day 7 and 14 after starting treatment. All follow up consultation for patients using insurances will include an additional 35$. Late cancellations (not cancelled 7 days in advance) and not show appointments will be billed in full. After initial evaluation, patient should have a follow up appointment in one month, and if no major adjustments needs to be done, the follow ups are expected every 3 months. Dr Zavala will be checking messages and answering questions through email in between.
